We know that insurance information can be confusing and hard to understand. That’s why, as a service to our patients, we research eligibility and verification prior to the patient’s first visit. We also directly bill insurance providers. If you have questions about our participation in a particular plan, please call us at 903-454-7555. Additionally, if you do not see your provider listed here, please contact us. While we try to maintain a comprehensive list, some information might change.
Patient Forms
In order to expedite your appointment and office visit, it is advisable to print the patient forms and have them completely and accurately filled out prior to your appointment. We suggest that you arrive approximately 15-20 minutes before your scheduled appointment if you have your forms filled out. If you do not have your forms filled out prior to your appointment, please plan on arriving 30 minutes before your scheduled appointment time. Please be prepared to provide us with your insurance card, identification, list of medications, and any necessary co-pays.

We gladly accept cash, personal check, Visa, MasterCard, American Express, and Discover.
Prescriptions
Prescription refills must be requested weekdays during normal business hours. Refill requests cannot be accommodated outside of regular office hours.

As your physician, we believe your right to privacy is a fundamental part of your treatment; as such, we want you to understand our privacy practices and procedures. Patient Education
Dr. Thomas believes that when you are an informed patient, you are your own best ally when facing or recovering from orthopedic surgery. He encourages you to participate by asking questions and doing as much research as possible about the procedures you are facing.
